Documentary Requirements

  • Citizenship Documents

    Important: The documents must be converted in pdf format except for 2x2 picture (only allowed format .jpg or .png)

    1. Philippine Statistics Authority (PSA) Birth Certificate;
    2. 2x2 ID Picture with nametag (white background);
    3. Certificate of Residency from the Barangay where the applicant reside;
  • Important: The documents must be converted in pdf format.

    At least one of the following:

    1. Form 138 for senior high school graduates.
    2. Accreditation and Equivalency Test Passer Certificate for an Alternative Learning System (ALS) graduate.
  • Important: The documents must be converted in pdf format.

    At least one of the following:

    1. Latest Income Tax Return (ITR) of one or both parents (if working)
    2. Certificate of Tax Excemption form Bureau of Internal Revenue (if non-working) with attached Affidavit of Low Income;
    3. Certificate of Indigence either from his/her Barangay or DSWD/MSSD;
    4. Case Study Report from DSWD/MSSD;
    5. Latest copy of contract or proof of income (for children of Overseas Filipino Worker (OFW) and seafarers);
  • Important: The documents must be converted in pdf format.

    At least one of the following:

    1. Single Parent ID from the Municipal or City Social Welfare and Development Office;
    2. Certification of IP Parents from the Ministry of Indigenous People’s Affairs (MIPA);
    3. Person with Disability (PWD) ID from the Municipal or City Social Welfare and Development Office;
    4. Senior Citizen ID of Parent/s or Guardian from the Municipal or City Office of the Senior Citizens Affairs (OSCA);
    5. Certifications of Death of Parents or Certification of Being an Orphan from the Barangay or Municipality/City;
    6. Certification of Being an IDP from the Barangay or Municipality/City;
  • Important: The documents must be converted in pdf format.

    At least one of the following:

    1. Certification from the MILF and MNLF Peace Mechanisms that the applicant is a former combatant or his/her children;
    2. Certification from MILF and MNLF Peace Mechanisms that the applicant is an orphan, his/her parent is a widow/widower due to the armed conflict, or is a surviving spouse;
